In order to process each admission smoothly, we would appreciate your assistance in obtaining the following information prior to admission:

  1. Dated report of recent chest x-ray (not more than 90 days old)
  2. Pre-admission physical examination (not more than 5 days old)
  3. Diagnosis and prognosis
  4. Adequate medical history
  5. Pertinent laboratory findings
  6. Current medical orders including medications, diet, ambulation and special considerations.
  7. Rehabilitation potential
  8. Level of care
  9. Pre-admission screen for mental illness and developmental disability
  10. Durable Power of Attorney or Guardianship Papers (copy required)
  11. Copy of Advance Directives
  12. Social Security Number, Medicare/Medicaid Numbers and other insurance policy numbers (copy of cards required)

If a resident is being admitted from the hospital the discharge planner will provide the facility with the above information.

If a resident is being admitted from home, the admission coordinator will assist you in obtaining the above information.

Admissions are done 24-hours a day, seven days a week.

The MDCH (Michigan Department of Community Health) prohibits the admission of a resident without the above information. It is designed as a protection to the other residents of the Center, not as an inconvenience to the resident. After it has been determined that admission is appropriate the Durable Power of Attorney or the guardian needs to contact the Center to make arrangements to complete the necessary paper work prior to admission.
